RESUMO

Dental anxiety is the second most common reason for patients to forgo treatment in France. Based on this observation, the odontology and psychiatry departments of Henri Mondor hospital proposed a collaborative approach to help these patients. The aim of this study is to evaluate the results of this pilot project over one year. The patients appreciated the consultation, considering it a tool to help them be able to receive care. This consultation also resulted in 17 percent of patients being offered psychological care in order to treat other problems related to dental anxiety. The idea of a network of practitioners specialized in the treatment of these anxious patients is therefore proposed, the patients having emphasized that not having to re-explain their anxieties would be a relief.


L'anxiété dentaire est la deuxième cause de renoncement aux soins en France. À partir de ce constat, les services de médecine bucco-dentaire et de psychiatrie du Groupe hospitalier Henri-Mondor ont proposé une prise en charge collaborative des patients concernés. L'objectif de cette étude consiste en un retour d'expérience sur une année de fonctionnement de ce projet pilote. Les patients qui ont bénéficié d'une prise en charge ont apprécié la consultation comme un levier pour être en mesure de recevoir des soins. Cette consultation a aussi permis de proposer une prise en charge psychologique pour 17 % des patients afin de traiter d'autres problèmes connexes à l'anxiété dentaire. L'idée d'un réseau de praticiens spécialisés dans la prise en charge de ces patients anxieux peut ainsi être proposée, ces derniers ayant soulignés leur soulagement de ne pas avoir à expliquer de nouveau leurs angoisses.

RESUMO

This paper reports the case of a 3-year-old male patient with triple teeth in the right maxillary incisor region and double teeth in the left mandibular incisor region. He had pre-existing medical conditions. The triple teeth were extracted and examined using micro-computed tomography. A literature review was performed to discuss this abnormality.

BACKGROUND: Children with first permanent molar affected by molar-incisor hypomineralization (MIH) show high treatment failure rate. AIM: To conduct a systematic review on bonding of adhesive materials to MIH-affected enamel, so as to identify all the methods suggested to optimize it and to determine the best bonding protocol(s). DESIGN: An exhaustive literature search was conducted on MEDLINE/PubMed, the Cochrane Library, and Web of Science databases, up to October 2018. Laboratory and clinical studies, involving adhesive restorations bonded to MIH-affected enamel, with at least a comparative group were included. Two authors independently selected studies, collected data, and assessed bias risk. RESULTS: After title and abstract review and duplicate exclusion, 14 articles were selected on the 496 eligible papers. After full reading, 4 articles were excluded. Finally, 10 studies (6 laboratory and 4 clinical studies) were included. CONCLUSIONS: Bond strength of composite was not significantly different when using self-etch compared with etch-and-rinse adhesives. Deproteinization after etching for etch-and-rinse adhesives enhanced bond strength; this could allow to keep MIH-affected enamel. Icon® showed an erratic penetration; however, a preliminary deproteinization after etching could improve bond strength. A study reported no significant differences in sealant retention rate, whereas another recommended to previously apply an adhesive.

This study aimed, after various delays of light-activation, to examine the microstructure of a resin-modified glass ionomer cement (RMGIC) by scanning electron microscopy (SEM) and to test its mechanical properties in short and long term. Five groups of Fuji II LC were investigated with different light-curing protocols. For each, SEM observations were undertaken with etched samples to selectively observe the organic polymer phase. Flexural strength and microhardness were also tested after 24 h and 12 months storage. SEM examination showed different images of the material according to the delay of light-activation. At 24 h, there was a delay with maximal mechanical properties: 5 min for flexural strength, 10 min for microhardness. However, at 12 months, maximal mechanical properties were reached for the shortest delay. The competition between the acid-base and polymerization reactions results in the formation of a structurally and mechanically different material according to the delay of light-activation. This delay should be adapted to the clinical situation.
